Facilities (Capital) Improvement Request Form

Authorization from Business Services is required before starting any site\facilities (capital) improvement project for district buildings, property, or grounds. A "Facilities (Capital) Improvement Project" includes any site or department improvement request that either adds something new to an existing facility or property, requires significant cost or resources to complete, is complex, or needs coordination among multiple departments.  The following is a list of possible projects that would be considered a capital improvement:  Adding an external window or door to a building, building out rooms or converting space into an office or meeting room, supporting school gardens with expansions or irrigation work, or demolishing and removing a fixed building.

Please note that the school or program administrator must discuss the project with Facilities to obtain a rough estimate of the project costs.  The FMOT Director will provide the projected project costs prior to the final review of the submitted project.  The school or program must identify the funding source.  Fiscal must approve district or grant-funded projects.

To submit your request, please complete and submit this form along with any necessary supporting documentation. Remember that certain types of facility improvements may also require Governing Board Approval and need to be a construction bid.
